Description

The Vanderbilt Assessment Scale is a 55-item scale used to aid in the diagnosis of ADHD, oppositional-defiant disorder, conduct disorder, anxiety, depression, and academic performance.

The ADHD subscale consists of the 18 items from the Vanderbilt - Parent that relate to ADHD, and the 8 items that relate to performance. It also includes 12 questions relating to side effects.

Special Instructions

1. Choose the answers that best describe the child's behaviors since the last time the assessment was filled out.
2. Once the patient has submitted it, you may preview it on the uninitialed documents tab in the EMR. However, do not initial it or it won't later be pulled into your Mobile Note.

ADHD Combined Inattention/Hyperactivity score (0-54)

Average Performance score (1-5)

Interpretation

Both scores are a measure of improvement over time with treatment
