Y-PSC - Youth Report Pediatric Symptom Checklist
Description
The Youth Report Pediatric Symptom Checklist is 35-item psychosocial screen, to be completed by the adolescent patient ages 11 and up, designed to facilitate the recognition of cognitive, emotional, and behavioral problems in children.
Special Instructions
Once the patient has submitted it, you may preview it on the uninitialed documents tab in the EMR. However, do not initial it or it won't later be pulled into your Mobile Note.

Total (0-70)
Interpretation
30 or higher may indicate psychological impairment
