Suicide Ideation Intensity Subscale

Description
Suicide Ideation Intensity subscale is a 5-item questionnaire culled from the Columbia Suicide Severity Rating Scale (C-SSRS). It is meant to quickly assess the severity of the the patient's suicidal thoughts and focus.

Special Instructions
Once the patient has submitted it, you may preview it on the uninitialed documents tab in the EMR. However, do not initial it or it won't later be pulled into your Mobile Note.
