Description
A 23-item measure that screens for the following DSM-5 domains: Depression; Anger, Mania, Anxiety, Somatic Symptoms, Suicidal Ideation, Psychosis, Sleep Problems, Memory, Repetitive Thoughts and Behaviors, Dissociation, Personality Functioning, and Substance Use.
Special Instructions
Once the patient has submitted it, you may preview it on the uninitialed documents tab in the EMR. However, do not initial it or it won't later be pulled into your Mobile Note.
Scores Produced
The measure produces a domain score for each of the following domains. The domain score represents the highest score on any item within the domain.
- Depression (0-4)
- Anger (0-4)
- Mania (0-4)
- Anxiety (0-4)
- Somatic Symptoms (0-4)
- Suicidal Ideation (0-4)
- Psychosis (0-4)
- Sleep Problems (0-4)
- Memory (0-4)
- Repetitive Thoughts and Behaviors (0-4)
- Dissociation (0-4)
- Personality Functioning (0-4)
- Substance Use (0-4)
Interpretation
For Depression, Anger, Mania, Anxiety, Somatic Symptoms, Sleep Problems, Memory, Repetitive Thoughts and Behaviors, Dissociation, and Personality Functioning, a domain score of 2 (Mild) or greater indicates additional inquiry and follow-up may be necessary
For Suicidal Ideation, Psychosis, and Substance Use, a domain score of 1 (Slight) or greater indicates additional inquiry and follow-up may be necessary
