PHQ-A
Description:
The PHQ-A is a 9-item self-report scale modified from the PHQ-9 for assessing the severity of depressive disorders and episodes in adolescents.
Special Instructions:
Once the patient has submitted it, you may preview it on the uninitialed documents tab in the EMR. However, do not initial it or it won't later be pulled into your Mobile Note.
