The PHQ-9 is a 9-item self-report scale consisting of the depression-related items from the Patient Health Questionnaire.

Once the patient has submitted it, you may preview it on the uninitialed documents tab in the EMR. However, do not initial it or it won't later be pulled into your Mobile Note.

Interpretation
Initial assessment:
- 1-4 Minimal depression
- 5-9 Mild depression
- 10-14 Moderate depression
- 15-19 Moderately severe depression
- 20-27 Severe depression
