Trauma Questionnaire
Description
The Trauma Questionnaire is non-validated version of the Life Events Checklist, and is designed to collect information related to the patient's experience with a wide variety of traumatic events.
Special Instructions
Once the patient has submitted it, you may preview it on the uninitialed documents tab in the EMR. However, do not initial it or it won't later be pulled into your Mobile Note.
